A Cardiovascular Intensive Care Unit (CVICU) Registered Nurse is an elite critical care specialist who manages the highly acute post-operative care of patients who have just undergone major open-heart, thoracic, or vascular surgeries.
Working in one of the most high-stakes environments in a hospital, CVICU nurses typically maintain a 1:1 or 1:2 patient ratio . They manage patients who are completely dependent on life-support technology and precise mechanical or pharmacological titration to survive.
Core Responsibilities

Immediate Post-Op Recovery:
- The "Fresh" Recover: They receive patients directly from the operating room (OR) while they are still under anesthesia and heavily unstable.
- Device Management: They closely monitor invasive lines and life-support systems, including mechanical ventilators, chest tubes, and temporary pacemakers.
- Advanced Life Support: They manage complex advanced mechanical circulatory support like ECMO (Extracorporeal Membrane Oxygenation), LVADs (Left Ventricular Assist Devices), and IABPs (Intra-Aortic Balloon Pumps).
Hemodynamic Monitoring & Titration:
- They read and interpret real-time data from Swan-Ganz (pulmonary artery) catheters, arterial lines, and central venous lines.
- They autonomously titrate powerful, continuous intravenous vasoactive "drips" (e.g., epinephrine, norepinephrine, milrinone) minute-by-minute to maintain strict blood pressure and cardiac output parameters.
Crisis Management & Early Intervention:
- They must recognize the earliest signs of lethal complications, such as cardiac tamponade, internal bleeding, or malignant arrhythmias.
- They are trained to perform emergency bedside procedures alongside surgeons, including emergency re-sternotomy (opening the chest at the bedside) if a patient arrests.
