This role combines clinical expertise with relationship management to ensure that patients transitioning from a hospital to home-based care do so safely and without interruption in their treatment. Your key responsibilities would include:Patient Assessments: Conducting clinical evaluations of referred patients to determine if they meet the specific eligibility criteria for home health or hospice services.Care Coordination: Collaborating with hospital case managers, discharge planners, and physicians to facilitate a seamless "handoff".Relationship Building: Maintaining professional partnerships with referral sources (doctors and social workers) to ensure the agency remains a top choice for patient placement.Marketing & Growth: Tracking referral activity and implementing outreach strategies to help grow the agency's patient census.Qualifications & SkillsLicensure: Licensed Practical Nurse (LPN)Experience: At least 1 year of experience in home health.Essential Skills: Strong communication, empathy for end-of-life care, organizational abilitySchedule This full‑time position follows a Monday-Friday schedule, 8:00 AM to 5:00 PM.Ready to Join Our Team?We value your time, so we've made our application process quick and simple.
