Registered Dietitian

Responsible for conducting assessments, developing and implementing individual care plans, and monitoring Adult Day Services participants with nutritional needs.

Essential Functions:

Conducts initial and 6-month nutritional assessments for participants referred by the Registered Nurses or Social Workers.

Develops and implements care plans based on the individual's nutritional needs, dietary history, food habits and preferences, and physician order.

Reviews the diet orders for participants on a quarterly basis and makes adjustments as needed.

Provides nutritional counseling on nutrition principles, dietary plans and food selection to participants and/or caregivers as needed.

Maintains appropriate records in participants' charts.

Oversees the provision of food services in coordination with the Office Manager and Self-Help's Nutrition Manager; advises on the preparation of therapeutic diets and evaluates food served at the center for conformance to quality standards and dietary prescription.

Provides in-service training for food service staff as needed.

Performs other duties as required.

Qualifications:

Currently registered by the American Dietetic Association.

Experience in working with the elderly or the chronically ill is strongly preferred.

Experience in working as a member of a multidisciplinary team is highly preferred.

Good organizational and communication skills.

Bilingual in English and Cantonese.

Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.

Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
