Registered Nurse Case Manager

Overview We provide in‑home care to former Nuclear Weapons Workers suffering from chronic and terminal illnesses.RN‑Case Managers conduct weekly nursing visits, perform physical assessments, oversee caregivers, and manage client care plans. Responsibilities Provide direct care and case management of assigned clients. Assist and collaborate with the regional director and personnel to identify and correct issues and improve services. Plan, implement, and evaluate care provided. Coordinate and manage client care conferences as needed. Serve as a local on‑site clinical resource and provide support to ensure client home‑care needs are met. Assist staffing coordinators with scheduling and staffing appropriateness. Adhere to nursing delegation guidelines and agency policies. Perform other functions as requested by the regional director, including interviewing, selection, ongoing evaluation of clinical personnel, training, education, and competency validation. Review clinical documentation for accuracy and completeness. Participate in agency performance improvement initiatives and collect, document, and submit data. Perform supervisory visits and review nurse shift reports. Assist in marketing activities such as visiting clients or physicians to discuss agency programs. Qualifications Graduate of a state‑approved school of professional registered nursing; BSN preferred. Current, unrestricted RN license in the state(s) of practice. Minimum of two years of nursing experience, including one year in home care or closely related field. Minimum one year of supervisory and/or case‑management experience preferred.
