Job Summary Location: Pueblo, CO Status: Part‑Time; Minimum expectation of 5 visits or 10 hours per week Hourly rate: $77 per visit Position: Home Health Speech Language Pathologist (SLP) We offer a compassionate, compliance‑focused environment verified by a deficiency‑free survey. Responsibilities Provide in‑house speech therapy to patients with communication, swallowing or cognitive difficulties due to illness or injury. Work as part of a home‑health team (RN, LPN, CNA, HHA, PT, OT) focused on the patient’s plan of care and established goals. Observe and evaluate a patient’s ability to communicate, swallow, and process information. Conduct patient assessments, document progress and ensure patient goals are met. Assist with mouth, breathing and swallowing exercises; fluency, articulation and reading interventions. Educate patient and family on plan of care, therapy goals, safety practices and self‑care. Qualifications Master’s or Doctoral degree in Speech‑Language Pathology and an active SLP license in Colorado. Minimum of one year of direct client‑care speech therapy experience within the last five years, preferably in home‑healthcare. CPR certification (demonstration course required). Knowledge of state and federal home‑health regulations. Strong clinical judgment, critical thinking, communication, empathy and compassion. No COVID‑vaccination requirement.
