* **Gross Weekly Pay:** $2,888.00/week
* **W2 Taxable Rate:** $26.00/hr
* **Weekly Non-Taxable Per Diem:** $1,848.00/week

## ⏰ Schedule

* **Shift:** Days
* **Shift Hours:** 08:00 AM – 04:30 PM
* **Schedule:** 5x8s
* **Shifts Per Week:** 5
* **Expected Weekly Hours:** 40 hrs/week
* **Weekend Rotation:** Once every 6 weeks

## Requirements

* Minimum **1 year Certified Home Health experience REQUIRED**
* Minimum **1 year Case Management experience REQUIRED**
* Ability to travel between patient cases
* Experience in hospice/private duty alone will NOT qualify
* Strong documentation and interdisciplinary communication skills required
