2 LPNs for Nights 6p- 630a 36hr GTE EOW Template 1 Week 1: Sunday Wednesday, Thursday Week 2: Monday, Friday, Saturday Holiday Group A Template 2 Week 1: Tuesday, Friday, Saturday Week 2: Sunday, Wednesday, Thursday Holiday Group B -How many beds in facility? 86 -How many units/halls? 3 Floors/Halls -How many beds on each unit/hall? 32 / 32 /22 -Average census of facility? 78 -Average Nurse to Patient ratios? 1:18 -Average CNA to Patient ratios? 1:8 -Typical residents/patients? Long-Term Care -Charting ? electronic or paper? Electronic, MATRIXCARE -Scrub color requirements? Blue or Black -What kind of OT do you think our travelers may be able to pick up for you? Minimal -Does your facility have any therapy animals on site? If so, what kind? No -Do you provide housing? If so, please give specific details on the housing. No, we can offer recommendations -If we provide housing, can you recommend any particular hotels/motels in the area? Alma Motel, Mount Pleasant Comfort Inn -What will orientation consist of? Full General Orientation and Competency Evaluation, tours, introductions, communication of our systems, enrollment into the EMR and Payroll System, Walkie-Talkie Distribution, Name Tags, etc. 8am, expect to be here 12 hours for training and shadowing. -What is your lunch break policy? Half hour unpaid plus three 15-minute PAID
