As part of our Home Health and Hospice team, you'll provide one-on-one care and become an important part of each patient's journey.

In This Role, You Will:

• Provide hands-on assistance with daily living activities
• Support patients with personal care needs
• Monitor patient well-being and report concerns
• Encourage comfort, safety, and independence
• Partner with clinical leadership to deliver exceptional care

Requirements

• Active CNA license
• Valid driver's license and proof of auto insurance
• Current CPR or BLS certification
• Current TB documentation (skin test within one year or chest X-ray within five years)

Additional Information

Care is provided in patient homes and residential facilities throughout Albuquerque and surrounding areas. Travel compensation is included within a 20-mile radius, with additional compensation opportunities available outside designated territories.
