Hospice RN Case Manager - Preceptor Home Health and Hospice - Waukesha

Job Summary Registered nurse plans, organizes, and directs/coordinates home care services with an emphasis on community health education. The professional nurse utilizes community resources to plan and direct services that meet the needs of individuals and families within their homes and communities. Duties & Responsibilities Develop written and individualized plans of care for each patient on the caseload; update and initiate plans of care and assume responsibility for ongoing interdisciplinary assessment. Assess patient condition, initiate and re‑evaluate plans of care, and update as necessary. Initiate preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care; provide skilled nursing care. Provide patient, caregiver, and family counseling and education. Prepare clinical notes and communicate with the responsible physician and other health‑care practitioners regarding the current plan of care. Obtain necessary physician orders and collaborate with physicians as patient needs dictate. Complete 75% of all charting in the patient’s home. Attend case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, and new problems. Re‑evaluate the need for continued care on an ongoing basis and initiate recertification when necessary. Identify discharge planning needs as part of care plan development and implement prior to patient discharge. Prepare clinical notes and update the primary physician at least every sixty days. Communicate with physicians regarding patient needs and report any changes in condition; obtain and receive physician orders as required. Coordinate the care plan with community health personnel. Additional Duties Participate in the agency’s quality assessment and performance improvement program and home‑health agency‑sponsored in‑service training. Participate in holiday rotation and on‑call duties as defined by policy and agency requirements. Ensure arrangements for equipment and other necessary items and services are available. Instruct, supervise, and evaluate home‑health aide care and LPN care provided every two weeks per state and agency regulations. Supervise LPNs, LVNs, HHAs/CNAs as directed. Participate in all mandatory meetings, training, and in‑service education. Complete all weekend/holiday visit documentation by the following business day at 8:00AM. Job Requirements Graduate from an approved school of nursing and be licensed in the State of Wisconsin. Experience in public health or home care nursing preferred but not mandatory. Licensed driver with an automobile insured per state or organization requirements and in good working order. Self‑directed and able to work with minimal supervision. Demonstrates excellent observation, problem‑solving, verbal and written communication skills and nursing skills per competency checklist. Shows ability to organize and prioritize workload independently. Responsible for supervising home‑health aides and LPNs. Physical qualifications: prolonged or considerable walking or standing; able to lift, position, or transfer patients; able to lift supplies and equipment; considerable reaching, stooping, bending, kneeling, or crouching; visual acuity and hearing to perform required nursing skills. Management experience not required.
