Nurse Practitioner Job Summary:- Conduct daily rounds on patients in a post acute care environment.
- Collaborate with a multidisciplinary team to assess and address patient needs.
- Develop and implement individualized care plans based on patient assessments.
- Monitor and manage patient progress, making necessary adjustments to treatment plans.
- Provide education and support to patients and their families regarding their healthcare needs.
- Maintain accurate and up-to-date patient records.
