Remote RN Care Coordinator III
Medica
Medica is seeking a Remote Care Coordinator in Minnesota to manage a caseload of members, develop individualized care plans, and coordinate delivery of medical and social services. You will educate members about Medicare and Medicaid benefits and act as a primary contact to address questions and concerns. Collaborating with physicians, providers, and county entities, you will discuss care plans, track member progress, and adapt plans as needed.
