What You'll Do:

As a private duty nurse on our home health team, you'll care for medically complex patients, primarily children, where they are most comfortable, at home.

• Provide personalized nursing care for pediatric and/or adult patients with complex needs
• Coordinate care plans with physicians, therapists, and other team members
• Monitor patient progress and update care plans as needed
• Educate and support families with in-home care techniques
• Document visits and care notes in our easy-to-use EMR (electronic charting system)
• Collaborate with your clinical supervisor and participate in case conferences
• Be a trusted part of a long-term care team focused on safety, comfort, and continuity

What You Bring:

• Active RN license
• CPR certification
• Clinical experience preferredbut we welcome passionate new grads, too
• Excellent communication and critical thinking skills
• A love for patient-centered care, especially in a home environment
• Confidence, compassion, and a calm presence during both routine care and critical moments
