The HHA works under the direction and supervision of a registered nurse./ppThis position will promote patients potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge./ph3Qualifications/h3pQualifications:/pulliCompletion of Certified Home Health Aide Program./lili1 year of Home Health Aide experience in a home health agency,/liliReliable transportation. Valid and current auto liability insurance./liliGood communication skills to be able to relate professionally and positively to patients, caregivers, and clinical team./liliAbility to read, write, and speak the English language/liliDemonstrates interest in the welfare of ill and elderly./liliProof of current, CPR and Hepatitis profile./liliAide Competency Evaluation Skills Checklist and Written Skills Test./li/ulpEssential functions:/pulliProvide direct patient care to patient under direction of the RN and according to the Aide Plan of Care./liliProvide necessary skills to assist the patient with safe transfers and ambulation per agency policy./liliProvide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care./liliPractice accepted infection control principles.
