RN Care Manager: Hospital Care Coordination & Transitions
Orlando Health
Orlando Health is seeking a Care Coordinator to promote effective management of hospital resources from admission to discharge. You will assess patients' risk factors, manage clinical utilization, and collaborate with clinical teams for optimal care coordination. The ideal candidate must be a graduate of an approved nursing school with a current Florida RN license and have at least three years of experience in care management or similar areas.
