divh2Registered Nurse/h2pConduct comprehensive nursing assessments of patients physical, emotional, and psychosocial needs./ppDevelop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides)./ppProvide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education./ppOffer education and emotional support to patients and their families/caregivers based on individual needs./ppSupervise LPNs/LVNs, home health aides, and other support staff./ppParticipate in on-call rotations, admissions, or urgent response as needed (common in hospice roles)./ppCoordinate care activities, including visit scheduling and documentation to ensure compliance with regulations./ppMonitor patient progress, respond to changes in condition, and adjust care plans accordingly./ppMaintain accurate clinical records and ensure care aligns with agency policies, professional standards, and regulatory requirements./ppGraduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation./ppOne (1) year experience as a RN./ppLicensed to practice as a registered nurse in the state of agency operation./ppMust possess and maintain valid CPR certification while employed in a clinical role./ppMust be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
