From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities. The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll. Licensed Practical Nurse (LPN) - Job Description

Responsible To: Registered Nurse Manager / Registered Nurse Supervisor

Function:

Provides on-site nursing services and health-related support to people at assigned sites. The LPN is responsible for promoting the health, safety, and well-being of people supported through the delivery of nursing care, monitoring of medical conditions, and implementation of physician-ordered treatments and services.

Working under the direction of a Registered Nurse, and in collaboration with the interdisciplinary team, the LPN ensures that all health-related services are delivered in accordance with applicable regulations, professional standards of practice, and agency policies and procedures.

Required Knowledge, Skills and Abilities:

Knowledge of nursing principles and practices within the scope of a Licensed Practical Nursing, including medication administration, infection control, and basic health assessment. Working knowledge of regulations and standards under the New York State Office for People with Developmental Disabilities, as well as applicable Department of Health and Medicaid requirements. Understanding of common medical, physical, and behavioral health conditions affecting people with developmental disabilities. Knowledge of person-centered planning and implementation of Individualized Plans of Protections, Plans of Nursing Services, and Care Plans. Familiarity with incident reporting requirements, including reportable events and immediate protective actions. Knowledge of documentation standards and electronic health records systems. Basic understanding of interdisciplinary team roles and coordination of care within a certified site-based setting.

Must have effective communication skills, including the ability to clearly convey medical information to non-clinical staff, people supported and their family members. Ability to provide training to support Direct Support Professionals in health-related tasks, including medication administration and infection control practices. Provides direct support to people as needed and as requested.

Minimum Qualifications:

Valid License as a Practical Nurse issued in the state of NY or a graduate of a Practical Nursing program. Must have a valid driver's license, acceptable driving record as established by the agency's insurance carrier.
