Nurse Practitioner / Physician Assistant

CenterWell Home Health is seeking a compassionate and clinically skilled Nurse Practitioner / Physician Assistant to deliver high-quality, in-home healthcare services to adult and senior patients. In this role, you provide comprehensive patient assessments, develop and adjust individualized care plans, and collaborate with an interprofessional team to support patients in achieving their highest level of independence and quality of life at home. You conduct in-home visits to evaluate medical histories, perform physical examinations, order and interpret diagnostic tests, and prescribe appropriate treatments within your scope of practice. You coordinate closely with physicians, skilled nurses, therapists, social workers, and caregivers to ensure continuity of care, timely follow-up, and safe transitions between care settings. Documentation is completed accurately and promptly within electronic health record systems, maintaining compliance with clinical protocols, regulatory requirements, and payer guidelines. You educate patients and families on disease management, medications, home safety, and preventive care, empowering them to actively participate in healthcare decisions.
