Create and monitor comprehensive care plans, adjusting interventions in real-time to drive positive patient outcomes and recovery. Coordinate multidisciplinary teams as the primary point of contact to ensure the patient receives a seamless, holistic treatment experience. Support the patient’s journey home by managing discharge planning early, ensuring a safe and confident transition from our care. Minimum Requirements Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Texas Board of Nursing required. Education Diploma from an accredited school/college of nursing required OR a bachelor’s level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
