About the Role:

The Community Health Worker_IAP plays a crucial role in bridging the gap between health providers and the communities they serve. This position focuses on improving health outcomes by providing education, resources, and support to individuals and families in need. The Community Health Worker will engage with diverse populations to promote health awareness and facilitate access to necessary services. By fostering relationships and trust within the community, this role aims to empower individuals to take charge of their health and well-being. Ultimately, the Community Health Worker_IAP contributes to the overall improvement of public health and the reduction of health disparities in the community.

Minimum Qualifications:

• High school diploma or equivalent.
• Experience in community outreach or health education.
• Strong communication and interpersonal skills.

Preferred Qualifications:

• Certification as a Community Health Worker.
• Experience working with diverse populations.
• Knowledge of local health resources and services.

Responsibilities:

• Conduct outreach to identify and engage community members in need of health services.
• Provide education on health topics, preventive care, and available resources.
• Assist individuals in navigating the healthcare system and accessing services.
• Collaborate with healthcare providers and community organizations to coordinate care.
• Collect and report data on community health trends and program effectiveness.

The hours of work are 28 per week. Required to travel to site locations occasionally.

This is a grant-funded position. Position is contingent on grant funding.

Skills:

The required skills of communication and interpersonal relations are essential for building trust and rapport with community members, enabling effective outreach and education. Organizational skills are utilized to manage multiple cases and coordinate with various healthcare providers and organizations. Problem-solving skills are crucial for addressing barriers that individuals may face in accessing health services. Preferred skills, such as knowledge of local health resources, enhance the ability to provide tailored support to community members. Overall, these skills contribute to the successful execution of responsibilities and the achievement of positive health outcomes.
