The Role:
The Supported Living Department at Chapel Haven Schleifer Center provides in-home support for individuals with intellectual, communication, or developmental conditions in the Westville neighborhood of New Haven, Connecticut, so that those served may lead a self-determined and independent life.
The Supported Living Program Manager is responsible for the overall quality of care within the Supported Living program. This position supervises Support Coordinators who provide direct-support services for individuals in their home setting. This includes oversight of staff schedules, continuity of care, while ensuring completion of required documentation, reports, and maintains high standards of person-centered services for the individuals that CHSC serves.
The SLP Manager will work closely with the Director of the program to fulfill the high quality of standards that the agency is respected for.
Essential Responsibilities:
- Supervision and Leadership
- Supervise and support a team of Support Coordinators, providing regular supervision, coaching, and performance evaluations.
- Ensure staff adhere to agency policies, state regulations, and best practices.
- Support team development through training and problem-solving.
- Program Oversight
- Ensure individualized supports are delivered in accordance with Person-Centered Plans, Individual Plans (IPs), including measurable goals that are in concert with the agency’s mission.
- Monitor the health, safety, and well-being of individuals served, intervening or communicating concerns as needed.
- Coordinate and convene with other departments (clinical, nursing, wellness, vocational, recreation, etc.) to ensure integrated services.
- Attend training, external or internal, to implement best practices as per the Director.
- Scheduling and Coverage
- Develop and maintain weekly staff schedules across community-based settings.
- Manage callouts, vacancies, and emergency coverage needs in collaboration with Support Coordinators and other Direct Support Professionals.
- Ensure the authorized / agreed upon weekly hours are consistently provided to the individual served.
- Documentation and Reporting
- Review and ensure timely, accurate documentation including progress notes, incident reports, doctor’s appointments and IP’s including measurable goals and objectives.
- Oversee submission of required 6 months and annual reports as required by state regulations or accreditation standards.
- Maintain current documentation for audits, inspections, licensing reviews, and quality assurance.
- Assist Support Coordinators with progress monitoring, data collection, annual reporting.
- Arrange and conduct annual meetings with families and stakeholders.
- Provide excellent communication between Support Coordinators, families, and Director.
- Work with vendors to keep them informed of necessary developments.
- Compliance and Quality Assurance
- Ensure program compliance with all relevant state, local, and agency regulations.
- Participate in audits, inspections, and quality service reviews (DDS QSR’s); develop and implement corrective action plans as necessary.
- Monitor service delivery to ensure alignment with agency values and person-centered practices
- Conduct internal quality service review and internal progress note audits
- On-Call and Emergency Response
- Participate in the on-call rotation for Supported Living programs.
- Respond to medical or personal emergencies during and outside of regular working hours.
Minimum Education Requirements:
- Bachelor’s degree in human services, Social Work, Psychology, Special Education, or related field (Master’s degree preferred).
Minimum Preferred Experience:
- Minimum of 3 years’ experience working with individuals with IDD, including supervisory experience.
Work Environment and Physical Demands:
- Office and community-based environments.
- Occasional lifting (groceries, luggage), stair usage (apartments), and walking are required.
- Must be available to respond to emergencies outside of regular hours.
- Driving individuals, locally and occasionally, outside of the Greater New Haven District.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Strong organizational and leadership skills with the ability to work independently and within a team.
- Experience with staffing coordination and schedule management preferred.
- Knowledge of Medicaid Waiver, DDS, and/or other relevant regulatory systems.
- Excellent communication, documentation, and problem-solving skills with emphasis on Positive Behavior techniques.
- Valid driver’s license and reliable transportation.
