Maintains contact with family and community agencies and makes referrals. Actively collaborates with team to meet patient care management and system improvement goals. Represents the behavioral health enterprise on institutional and community committees/task forces.## We’re seeking a Licensed Mental Health Professional (LCSW or LPC-MHSP) to join our new outpatient service line supporting Ambulatory ECT patients. This hybrid role blends direct patient care and administrative responsibilities care coordination and program support. The clinician will conduct initial patient assessments, develop treatment plans, and coordinate therapy and resources as part of a collaborative team. The position offers a flexible hybrid schedule after 90 days.## KEY RESPONSIBILITIES* Provides initial assessment and develops plan of care according to TN Department of Mental Health, The Joint Commission and CMS Best Practice Guidelines and within the scope of their license by the Tennessee Health Related Boards.* Provides targeted evidence based interventions into clinical practice with patients.* Facilitates transitions to appropriate levels of care via in person, telephone or telehealth contact.* Acts as clinical liaison with patient, family, treatment team and community.* Contributes to the organization by actively participating in process improvement initiatives and shared decision making.* Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards.* Maintains professional documentation which meets department and VUMC standards as it relates to compliance.* Provides leadership and supervision within scope of license to meet departmental needs.* Maintains confidentiality and privilege in accordance with TN state codes and statutes* Upholds ethical standards in accordance with relevant professional association’s code of ethicsTECHNICAL CAPABILITIES* Crisis Intervention (Intermediate): Demonstrates mastery of crisis intervention in practical applications of a difficult nature. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.* Patient Documentation (Intermediate): Demonstrates mastery of patient documentation in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering patient documentation services without requiring support and instruction from others.
