Community Access Inc
Community Access Inc. seeks a Peer Health Navigator for its Intensive Crisis Residence (ICR) program in New York City. The role focuses on linking guests to community resources during crisis and arranging discharge planning with ongoing navigation for up to 90 days after discharge.
