Patient Care & Advocacy Start‑of‑Care Excellence: Meet new patients, explain services, and complete comprehensive start‑of‑care assessments. Individualized Care Planning: Develop and manage personalized plans of care that respect medical needs, patient preferences, and family dynamics. Ongoing Clinical Oversight: Monitor patient progress, adjust care plans, and communicate changes with physicians and interdisciplinary teams. Family Education & Support: Teach and supervise family members involved in patient care, ensuring safety, confidence, and continuity. Team Leadership & Supervision Caregiver Coaching: Guide nurses, aides, and personal care staff on care plans, best practices, and expectations. Performance Oversight: Review documentation, monitor attendance and reliability, and provide timely feedback and coaching. In‑Home Supervisory Visits: Observe care in action to ensure quality, compliance, and patient satisfaction. Training & Orientation: Lead or support onboarding and ongoing education aligned with company standards and values. Compliance & Quality Assurance Regulatory Excellence: Ensure compliance with OSHA, and state licensure requirements. Clinical Documentation: Maintain high standards for records, progress notes, and compliance reporting. Quality Improvement: Participate in interdisciplinary meetings and initiatives that elevate clinical outcomes and service quality. Community Engagement & Growth Relationship Building: Develop strong partnerships with referral sources, healthcare providers, and community organizations. Brand Advocacy: Represent Capital City Nurses and Care Advantage in the community, modeling iCARE values. Growth Support: Contribute to organizational growth, mergers, and acquisitions while maintaining consistency in care standards.
