Field Care Manager: Care Coordination for High-Need Members
Molina Healthcare
Molina Healthcare seeks a Care Coordinator to support care management for LTSS. You will coordinate with a multidisciplinary team to deliver integrated member care, including home visits when required, and develop waiver service plans with members, caregivers, and providers. The role requires 2+ years in health care, LTSS or chronic conditions experience, a valid driver’s license, and ability to travel locally (25-40%).
