Highlights Pay: $63,250.00 - $96,467.50 Location: Cleveland, OH Facility: Cleveland Clinic Main Campus Department/Unit: Orthopedics Schedule: Full-time, Day shift What You'll Do Work collaboratively with a multidisciplinary care team for high-risk patients. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments covering various patient needs. Inform and work with patients and families regarding care coordination. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources. Requirements Education Graduate from an accredited school of professional nursing. Experience 3-5 years of nursing experience. Licenses & Certifications Current state licensure as a Registered Nurse (RN). Basic Life Support (BLS) Certification through AHA or American Red Cross. Bachelor of Science in Nursing (BSN) preferred.
