Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC

Forsyth offers convenient access to central Illinois destinations with a quieter suburban setting just north of Decatur. Hickory Point Mall and Hickory Point Golf Course are both located in Forsyth, and downtown Decatur is about 15 minutes away for dining and entertainment.
