While the majority of the work can be performed from your home office, you will maintain strong connection with our healthcare team, participating in regular virtual meetings and collaborating on patient care strategies.   • Conduct thorough psychosocial assessments by engaging patients in insightful interviews and analyzing medical records to identify emotional, social, and environmental factors that may impact recovery.

  • Provide critical counseling and emotional support to patients and families, navigating adjustment issues related to rehabilitation, and implementing effective coping strategies.

  • Work collaboratively with a multidisciplinary rehabilitation team to design and implement individualized treatment plans tailored to align with each patient’s psychosocial needs.

  • Empower patients by facilitating access to essential community resources, including support groups, vocational rehabilitation services, and housing assistance programs.

  • Educate patients and their families about available community resources and post-rehabilitation services to promote continued support.

  • Participate actively in discharge planning, ensuring smooth transitions to home environments or alternative care settings.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at our facility, you will support a diverse array of patients recovering from various conditions, including strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your role will significantly impact their rehabilitation journey, facilitating their emotional well-being and guiding them towards successful reintegration into the community.

  • A Master’s degree in Social Work from an accredited program is required.

  • Must hold an active state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
