Kelsey-Seybold Clinic, part of Optum, is seeking an ED RN Case Manager in Pearland, TX. You will monitor real-time ADT notifications and coordinate discharge planning to support seamless transitions of care across hospitals, PCPs, specialists, and home care teams. The ideal candidate holds an undergraduate nursing degree, an unrestricted RN license, and at least four years of diverse clinical experience.
