Prime Staffing is seeking a travel nurse RN Home Health for a travel nursing job in San Diego, California.Job Description & RequirementsSpecialty: Home HealthDiscipline: RNStart Date: 07/13/2026Duration: 13 weeks40 hours per weekShift: 8 hours, daysEmployment Type: TravelWe are seeking a compassionate and experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes in San Diego, CA.Key ResponsibilitiesPerform skilled nursing visits and comprehensive in-home patient assessments.Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.Develop, implement, and update individualized care plans.Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.Educate patients and caregivers on disease management, medications, and home safety.Coordinate care with physicians, therapists, case managers, and other healthcare professionals.Maintain accurate and timely documentation in the EMR.Minimum RequirementsMinimum 1 year of recent Home Health RN experienceExperience caring for geriatric patientsCurrent BLS/CPR CertificationOASIS experience requiredActive California RN License preferred (pending CA license accepted if application has been submitted prior to submission)Valid Drivers License, reliable transportation, and current auto insuranceThe ideal candidate is a compassionate and organized Home Health RN with recent OASIS experience who enjoys providing personalized, high-quality care in patients homes while effectively managing an independent schedule.About Prime StaffingAt Prime Staffing, we understand the importance of finding the perfect fit for both our clients and candidates.
