Major Responsibilities Provide ongoing assessment, planning, implementation and evaluation of customer needs to enable proactive planning of admissions, discharges and transfers. Perform review of potential admissions (ED, direct, procedural, or transfers between facilities) to match the patient’s health‑care needs with the most appropriate level of care. Collaborate with the health‑care team, including the attending physician and transferring facility, regarding medical criteria for admission and appropriate level of care based on accepted medical‑necessity criteria, standards, evidence‑based practice and current research. Assist the physician in identifying alternatives to admission and appropriate referrals for patients that do not require acute‑care services. Refer denials or cases that do not meet medical‑necessity criteria to a physician advisor. Maintain and accurately use patient and bed‑management data for trending and planning. Participate in efforts to reduce risk and improve patient safety. Think critically and process information in a high‑pressure environment. Demonstrate adaptive communication styles based on the needs of each situation. Establish and maintain positive customer relationships with all internal and external clients. Resolve complaints promptly using service‑recovery strategies and address conflicts emerging between patients, families and health‑care providers. Facilitate and coordinate assignment of patients to the most appropriate bed. Maintain an overview of capacity within Advocate Health Care and match bed requests with available resources, maximizing appropriate utilization of resources. Anticipate bed needs and create assignment plans by comparing available beds with surgery and cath‑lab schedules and anticipated admissions from ED, physician offices and other facilities. Collaborate with multiple departments (ED, surgery, PACU, cath‑lab, nursing) to facilitate patient throughput. Use available resources to make informed bed‑placement decisions, incorporate infection‑control guidelines, and maintain communication with bed‑management support staff. Oversee and drive the bed‑management process through regular rounding on units, collaborating with unit staff on discharges, admissions and bed placements. Maintain authority to compel units to accept new patients and explain clinical reasoning and operational constraints, demonstrating detailed clinical knowledge of patient‑care needs. Collaborate with physicians and charge nurses to assess care level for ICU, telemetry and step‑down patients who can be moved to medical/surgical units; identify and establish a code bed. Communicate bed‑flow status and census to leadership, follow bed‑management protocol and mobilize resources to resolve throughput disruptions. Manage an electronic bed board and reservation process for scheduled and unscheduled admissions, discharges and transfers. Provide education to the health‑care team regarding admission criteria and patient‑placement guidelines. Apply admission criteria to all potential admissions to evaluate appropriateness based on severity of illness and intensity of services. Ensure appropriate hospital utilization in accordance with health‑and‑human‑service regulations, payment‑class requirements and accreditation standards. Document bed‑management activities, including admissions, transfers, pre‑arrival and consult‑call data in designated software systems. Use evidence‑based practice to drive improvement strategies. Collaborate with leaders and team members to ensure operational efficiency of patient throughput and positive patient outcomes. Understand departmental and organizational productivity measures; assess productivity and recommend resource adjustments. Utilize admission, transfer and discharge data to support proactive decisions with consideration of workflow, staffing and budgeted census. Lead initiatives to develop, implement and track quality‑improvement activities, especially related to nursing‑sensitive indicators. Act as a professional role model; hold self and peers accountable for advocating service and patient‑safety behaviors with zero tolerance for deviation from standards. Review available resources to maintain knowledge of department and hospital activities. Collaborate with associates to facilitate teamwork and demonstrate pride in achieving targeted outcomes. Demonstrate effective and courteous interdepartmental communication to facilitate house‑wide patient movement. Consistently project a positive professional image through appearance and behavior; use appropriate communication skills for collaborative problem solving and planning. Work independently, make responsible decisions, and demonstrate time‑management and organizational skills on a routine basis. Maintain required contact hours for licensure and stay current with standards of care via literature review and educational offerings; achieve professional certification as required. Attend staff meetings regularly and complete required hours based upon FTE status within the specified timeframe. Licensure Registered Nurse (RN) Education / Experience Required Bachelor of Science in Nursing (BSN) or a combination of 1‑2 years of logistics (patient placement/bed management) experience and 2‑3 years of clinical nursing experience. Knowledge, Skills & Abilities Required Ability to prioritize work. Effective interpersonal communication skills. Problem‑solving and timely decision‑making. Strong critical‑thinking skills. Competence in computer applications and the ability to multitask between software systems. Knowledge of MS Office suite. Current nursing license in the State of Illinois. CPR certification. Physician Requirements And Working Conditions Ability to prioritize independently and respond to multiple simultaneous requests. Ability to work in a fast‑paced environment with established time constraints and challenging situations. Availability to work flexible hours, including a 24‑hour, 7‑day per week schedule for the Patient Command Center. Ability to communicate with site leadership during critical high‑census surges to facilitate discharges and admissions. Ability to lift up to 35 pounds without assistance. #J-18808-Ljbffr
