Job Title: RN Case Management Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 5 years recent RN Case Management experience, with experience in long-term acute care or short-term acute care settings, discharge planning across acute care settings, and strong interdisciplinary team collaboration. License: Must have a Texas RN or multi-state RN license. Certifications: BLS certification through the American Heart Association is required. Must-Have: - Comprehensive assessment and care planning skills. - Experience in care coordination and evaluation of individual patient needs. - Patient advocacy skills in resource utilization and discharge planning. Description: Coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, and plan implementation. Enhances the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness. Integrates functions of case management, utilization review, and discharge planning. Delivers ongoing support and expertise while ensuring patient preferences are met. Serves as a patient advocate through resource utilization and addressing the holistic needs of the patient.
