Description

• Conduct patient assessments in both inpatient and outpatient environments to provide tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work in collaboration with interdisciplinary teams including nurses, physicians, and other rehabilitation specialists to enhance patient outcomes
• Educate patients and their families about therapeutic objectives, discharge planning, and at-home exercises
• Meticulously document evaluations, treatments, and patient progress in accordance with facility and regulatory protocols
• Engage in care team meetings and interdisciplinary conferences to facilitate cohesive care strategies

Education

• Doctor of Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory
