JOB SUMMARY:

The Nurse Coordinator is responsible for coordinating the care of all Think patients. Serves as the main conduit of the provider's entire clinical team. This role is assigned to a care team and works closely with a multidisciplinary team to provide the best personalized care to the patients ensuring that the appropriate levels of healthcare resources are utilized. This role also develops a patient-specific plan of care for patients enrolled in a specific value-based program. The Nurse Coordinator will assist each patient in following their plan of care to improve health outcomes and satisfaction and to decrease healthcare costs.

ESSENTIAL JOB FUNCTIONS:

1. Assess the medical, social, and psychological needs of patients.
2. Create patient-specific care plans and identify which resources to deploy for each patient.
3. Provide education on various healthcare conditions to improve health outcomes for patients and families.
4. Maintain detailed documentation in patients' Electronic Health Record.
5. Coordinate with various service providers to ensure the fulfillment of the patients' needs.
6. Correspond with patients via phone using internal protocols, to provide appropriate recommendations for needed care.
7. Collaborate with providers in planning routine or acute visits and obtain the required information before the visit.
8. Review outside clinical data, e.g., labs, x-rays, and consultation reports, and delegate to the proper team member
9. Coordinate patient specific care with nursing homes, home healthcare agencies, and hospitals as needed.
10. Make outgoing calls to complete transition of care visits and schedule appointments as needed.
11. Delegate tasks appropriately to LPN, MA, and other unlicensed staff.

KNOWLEDGE, SKILLS & ABILITIES:

1. Knowledge of patient care standards and practices.
2. Knowledge of scope of practice and patient care standards/practices.
3. Knowledge of medical terminology.
4. Knowledge of the resources available for patients and the requirements needed to access those resources.
5. Knowledge of HIPAA privacy rules.
6. Skill in communicating in a professional manner, both verbally and in writing.
7. Skill in planning, organizing, and time management.
8. Skill in using a computer and a variety of software, including Electronic Health Records (EHR) software, Word, Excel, and Outlook.
9. Skill in managing multiple priorities and problem solving.
10. Ability to work effectively under pressure to meet established goals and objectives.

12. Ability to maintain high levels of professional and personal integrity.
13. Ability to convey professional demeanor and sound judgment in dealing with patients' care, protection of patient privacy, and confidentiality.

EDUCATION & EXPERIENCE:

Associate Degree in Nursing (ADN) required, Bachelor of Science in Nursing (BSN) preferred. Active Nursing License in the State of Nebraska required. Active BLS/CPR certification required. A minimum of two (2) years of nursing experience required. Bachelor of Science in Nursing (BSN) and two (2) years of experience in case/care management working with adult patients preferred. One (1) year of experience in a triage nursing setting preferred.

WORKING CONDITIONS AND PHYSICAL EFFORT:

• This role operates in a healthcare setting. This position requires frequent sitting and computer work and allows employee to vary physical position or activity for comfort.

• Must be able to:

• Stand 10% or longer of a workday.
• Walk 10% or longer of a workday.
• Sit 80% or longer of a workday.

• Requires ability to lift up to 10 pounds occasionally.

• Requires employee to: bend, squat, kneel, and reach above shoulder level occasionally and twist occasionally.

• Requires repetitive use of hands for: simple grasping, fine manipulation, computer use.

• Requires all sensory skills (speech, vision, smell, touch, and hearing) corrected to near normal range.
