Community Health Advocate — In-Home Care & DME Delivery

Your Health Organization in Georgia is seeking a dedicated Community Health Worker to serve patients in Johns Creek, Duluth, and surrounding areas. The CHW acts as a bridge between patients, healthcare services, and community resources, visiting homes and ALFs to coordinate visits with providers and ensure timely access to care. Responsibilities include delivering medications and DME, coordinating telehealth visits, training on RPM equipment, venipuncture and specimen collection, and supporting #J-18808-Ljbffr Your Health Organization
