Registered Nurse – Home Care Two Openings: Nashua, NH & Greater Londonderry, NH $12,500 sign-on bonus! Our client has been a trusted provider of home health, hospice, and palliative care for over a century. They have two (2) Home Care RN openings — one serving Nashua, NH, and one serving the Greater Londonderry, NH area. We are seeking an RN with at least one year of home care, acute care med/surg or acute rehab experience. Responsibilities Completes an initial home health assessment of patient and family to determine home care needs; provides a full physical assessment and history of current and past illnesses. Utilizes the nursing process in planning, providing, and evaluating skilled needs. Completes a comprehensive patient assessment reflected in accurate OASIS scoring. Develops a care plan with goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes patient and family in planning and revising care. Case manages your patients—you are key to helping them heal and return to strength and independence! Required Graduate of an accredited school of nursing. Associate degree in nursing. At least 1 year of recent home care or acute care experience (preferably med/surg or acute rehab). Ability to exercise initiative and independent judgment. CPR certification. Satisfactory criminal record. Satisfactory motor vehicle record (travel between patients is reimbursed). Preferred Bachelor’s degree in nursing. Community Health/Education experience. Additional certifications (IV, Chemotherapy, Ostomy, etc.). Familiarity with OASIS and reimbursement drivers. EMR software proficiency.
