Transplant Coordinator Nursing
12-hour variable shifts, MondayFriday, 7:30 AM 5:00 PM; no major holidays or weekends
3+ years of transplant coordinator experience preferred
Strong ambulatory nursing experience required
Experience coordinating pre- and post-transplant care, reviewing labs, patient education, triage, multidisciplinary coordination, and managing medically complex patients
Experience with kidney transplant population and Epic preferred
BLS certification required
Screen patients for transplant eligibility and coordinate necessary testing and appointments
Provide patient education regarding pre- and post-transplant care
Manage medication protocols for transplant patients
Collaborate with multidisciplinary teams to ensure comprehensive patient care
Monitor and follow up on patients' clinical progress throughout their care journey
