Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, followed by tailored treatment plans
• Deliver physical therapy interventions focusing on enhancing mobility, strength, balance, and overall functional capabilities
• Work in tandem with nurses, physicians, case managers, and various rehabilitation professionals to achieve optimum patient outcomes
• Educate patients and their families about therapeutic goals, planning for discharge, and exercises to undertake at home
• Document evaluations, treatments, and progress adhering to facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure harmonized patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
