Clinical Liaison: Admissions & Care Coordination

Lifepoint Rehabilitation seeks a Clinical Liaison to connect referring facilities with our hospital admissions teams. You will assess patient needs, coordinate intake and pre-admission screening, and determine admission appropriateness with on-site clinical judgment. You will communicate options to patients and families and secure information on resources and benefits. Applicants should hold clinical licensure (RN, PT, OT, SLP), have EMR experience, a valid driver's license, and strong #J-18808-Ljbffr Lifepoint Health®
