Guaranteed Hours: 40 hours per week

Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities
- Assess patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure high-quality, continuous care.
- Educate patients and family members regarding care plans, medications, and safety practices.
- Supervise and provide clinical direction to home health aides, ensuring quality care delivery.
- Address pain management and educate patients and families on techniques to manage pain effectively.
- Communicate effectively with patients, families, and interdisciplinary team members to coordinate care.

Compliance / Notes
- On-call responsibilities may be required.
- Orientation and compliance checks may be mandated.
