Position Summary We are seeking an experienced RN Case Manager to work full‑time on a compassionate interdisciplinary team dedicated to patient‑ and family‑centered hospice care. Key Responsibilities Complete the nursing assessment of assigned patients and maintain an ongoing assessment within hospice guidelines. Collaborate with core disciplines—medicine, social work, and spiritual care counseling—to meet care goals. Coordinate the provision of medications, durable medical equipment, and disposable supplies, ensuring timely and cost‑effective delivery. Apply universal precautions in all direct patient care. Provide ongoing professional nursing care, regularly assess and reevaluate patient and family needs, and report findings to supervisors and the interdisciplinary team. When patient or family needs change, communicate with the attending physician or designee, obtain and document necessary orders, and collaborate with the supervisor on changes in level of care. Provide instructions regarding medication, activities, diet, symptom management, safety, expected disease progression, and the dying process in accordance with the patient’s plan of care. Ensure all patient documentation is complete, accurate, and reflects nursing assessments, interventions, and goals within 24 hours of patient visit. Complete or update patient plan of care, ensuring problems, plans, actions, and goals are accurately stated and written per federal, state, and YoloCares standards. Qualifications Current and active Registered Nurse (RN) license in the State of California. Clear verbal and written communication skills in English; Spanish language skills are a plus. Minimum of two (2) years of recent clinical nursing experience; experience in home care or hospice care highly desirable. Demonstrated clinical nursing skills and commitment to the philosophy, principles, and goals of hospice care. Compensation Pay range: $114,000 – $124,800 annually.
