RN Case Manager - Home Health

Location: Bradford, VT area, Randolph, VT area, Springfield, VT area, White River Junction, VT area, Lebanon, NH area

Assignment Duration: This is a 13-week contract position.

Schedule: Full Time, Days

Job Description: As an RN Case Manager at Visiting Nurse and Hospice for Vermont and New Hampshire, you will interact closely with patients, becoming an integral part of their care team. Enjoy a flexible work/life balance while providing exceptional nursing, rehabilitation, hospice, and personal care services.

Responsibilities:

• Develop care plans in collaboration with the patient's physician, the patient, and their caregivers upon admission.
• Periodically review and recertify the patient care plan as necessary.
• Coordinate the care plan through interdisciplinary communication and case conferences.
• Advise other disciplines of discharges in a timely manner.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status.
• Document evaluations, treatment goals, and objectives accurately.
• Teach self-care techniques to patients and caregivers.
• Provide medication, diet, and other instructions as ordered by the physician.

Required Experience / Certifications / Licensure:

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• 2 years of experience in Home Health.
