Optum

Elite Home Health, part of the Optum family, seeks an LPN to provide quality home health care in patients' homes in Missouri. The LPN collaborates with the health care team, documents visits, and participates in care coordination. This role requires an active LPN license in the state, CPR, a valid driver's license, and flexibility to meet patient needs.
