Position Summary We're looking for an experienced RN Case Manager to join our clinical team. This role is primarily field-based, conducting patient assessments and re-assessments in clients' homes throughout Chicago, while managing an active caseload and serving as a clinical resource for our field nursing staff. The ideal candidate combines strong critical care clinical judgment with the flexibility and independence needed to manage a mobile, client-facing schedule. Key Responsibilities Conduct comprehensive new patient (start of care) assessments in clients' homes Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes Ensure care plans and documentation meet regulatory and Joint Commission compliance standards Identify and upscale clinical concerns or changes in condition appropriately Participate in office-based team meetings, case conferences, and administrative work (~1 day/week equivalent) Qualifications Required: Active, unrestricted RN license in the state of Illinois Critical care clinical experience Reliable personal vehicle, valid driver's license, and current auto insurance Comfortable with extensive daily driving across the greater Chicago area Negative TB Test and Physical Current CPR Certification Strong assessment, critical thinking, and clinical decision-making skills Excellent communication skills — comfortable being a clinical resource for peers Proficiency with electronic documentation systems Preferred: Minimum 1 year of ICU experience Home care, hospice, or case management experience What We Offer Competitive salary + mileage reimbursement Benefits Package including Health, Dental & Life Insurance. 401(K) with match.
