Licensed Practical Nurse (LPN) – Day ShiftAvondale Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Humboldt, Tennessee. In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility.LPN RequirementsActive Licensed Practical Nurse (LPN) license in TennesseeExperience in long-term care, skilled nursing, or rehab preferredStrong clinical and assessment skillsExcellent communication and teamwork abilitiesAbility to prioritize and work in a fast-paced environmentWillingness to work weekends and holidays as neededLPN ResponsibilitiesProvide direct nursing care according to individualized care plansAdminister medications and treatments as orderedMonitor resident condition including vital signs and changes in statusComplete accurate and timely documentationAssist with admissions, transfers, and dischargesWork closely with RNs, CNAs, physicians, and therapy staffSupport residents with daily care needs when neededParticipate in care planning and family meetingsFollow infection control and safety proceduresHelp train and support new nursing staffWhy Join Avondale Health and Rehabilitation CenterWe believe in supporting the people who care for our residents every day.
