About the Position

Job Title: Acute Care Social Worker Profession: Behavioral Health Specialty: Clinical Social Worker Duration: 13 weeks Shift: 5x8 or 4x10 M-F days Hours per Shift: 40 Experience: Minimum 2 years in transitional care in a hospital setting License: BSW or MSW in social services required Certifications: BLS preferred Must-Have: Minimum 2 years of experience in a hospital environment in social work. Must be at least 21 years of age. Description: Minimum 2 years of experience in transitional care in a hospital setting is required. Flu vaccine is required. Fingerprint cards are required to be submitted within 1 week of offer for background clearance. This position works closely with the inpatient multidisciplinary care team. It provides proactive integrated social services care management that supports the biopsychosocial needs of patients. The patients range from birth to end of life. This position works directly with patients and their families. Support is provided in planning for discharge from the hospital. Barriers to discharge are identified. Assistance with financial and housing needs is provided. Work is done with state and federal agencies to provide information for placement needs. Custodial issues are identified, and referrals are made for patients in crisis to behavioral health. This position handles referrals and consults from providers in primary care and outlying clinics. Responsible for all swing bed admission assessments and patient disposition. Supportive counseling, encouragement, coaching, and linkage with community services are provided. Essential functions include: Working closely with the inpatient multidisciplinary team as a member of the care coordination team. Assessing the biopsychosocial needs of any patient admitted for medical care. Responsible for discharge planning in collaboration with the inpatient care coordinators. Identifying barriers to discharge. Documentation and identifying community resources and agencies needed at discharge. Identifying Durable Medical Equipment (DME) needs. Developing discharge plans in agreement with patients and/or families/caregivers. Completing swing bed assessments at admission into subacute rehabilitation services. Ensuring patients and/or families understand the level of care and participation requirements. Facilitating care conferences to ensure patient and family knowledge of illness and care plans. Assisting patients in making healthcare decisions, involving family or caregivers as desired. Identifying community resources available for assistance during hospitalization and recovery. Submitting requests that may support meeting basic living requirements. Assisting patients with financial or legal matters. Supporting patients with long-term care or assisted living needs. Finding alternative solutions to meet the physical and emotional needs of each patient. Assisting with dental, podiatric, eye care, and hearing services while patients are hospitalized. Referring to a social service clinician for chemical dependence, mental health, and crisis support. Maintaining complete and accurate records as required. Participating in multidisciplinary team meetings. Participating in staff/team training and incorporating concepts into patient services. Establishing and maintaining up-to-date knowledge and effective relationships with local community resources. Developing trusting relationships with assigned patients. Supporting provider decision-making and building on provider-patient interventions. Coordinating with team members to ensure the continuum of care. Obtaining complete and accurate information in a courteous and culturally sensitive manner. Documenting patient notes clearly and timely to provide adequate information for other providers. Successful candidates will demonstrate an understanding of ethical practice and case management principles. Knowledge of community support resources and customer service skills is essential. Communication skills are necessary to interact positively and effectively with patients and families. The ability to maintain strict confidentiality of patient care information is critical. Candidates must be able to multi-task and work independently in a dynamic environment. Strong organizational skills and the ability to meet deadlines are important. Collaboration with other state and community agencies on behalf of patients is required. Advocacy and outreach skills are essential to succeed in this role.
