RESPONSIBILITIES: Assess the psychosocial status of patients/families/caregivers related to the patient's illness and environment Carry out social evaluations and plan interventions based on evaluation findings Counsel patient/family/caregivers as needed in relationship to stress and other identified coping difficulties Maintain clinical record on all patients referred to social work Provide information and referral services for organization patient/family/caregivers regarding practical and environmental needs Maintain collaborative relationships with organization personnel to support patient care Maintain and develop contacts with public and private agencies as resources for patient and personnel Participate in discharge planning when needed REQUIREMENTS: License in Social Work preferred Experience in Long Term Care preferred Start your fulfilling career with the Quality Life Services family!
