Starting Salary: $120,000 annually

Position Overview
We are seeking a kind, compassionate, and motivated Nurse Practitioner to join our growing team. This role focuses on providing primary care services and palliative care consultations to geriatric patients across a variety of care settings. We are open to new graduates who are eager to learn and grow, as well as experienced providers.

The ideal candidate is passionate about caring for seniors, committed to delivering high-quality care, and interested in developing a long-term career within a supportive and expanding organization.

Key Responsibilities

• Provide comprehensive primary care services to geriatric patients
• Conduct palliative care consultations with a focus on comfort, dignity, and quality of life
• Deliver care in assisted living, subacute rehab, long-term care, and home settings
• Collaborate with facility staff, families, and interdisciplinary care teams
• Develop and implement individualized care plans
• Maintain accurate and timely documentation
• Participate in ongoing learning and professional development

Qualifications

• Active Nurse Practitioner license in the state of New Jersey
• DEA license and CDS (Controlled Dangerous Substances) registration, or eligibility to obtain
• Ability to prescribe medications in accordance with state and federal regulations
• Strong clinical assessment and diagnostic skills
• Excellent communication and interpersonal skills
