Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is hiring a Service Facilitator (SF) Case Manager in Virginia. You’ll visit clients in their homes, guide families through Medicaid Waiver programs, and write care plans that truly help people live independently and safely. Role requires at least 2 years working with individuals with disabilities or elderly, no degree required, and a valid driver’s license with reliable vehicle.
