Responsibilities
- Perform comprehensive physical therapy evaluations and re-evaluations for residents in the skilled nursing setting.
- Develop, implement, and modify individualized treatment plans focusing on functional mobility, gait, balance, strength, and activities of daily living.
- Apply evidence-based therapeutic interventions and modalities within scope of practice; document progress accurately and timely.
- Monitor patient response, adjust treatment plans as needed, and communicate with the interdisciplinary team including physicians, nursing, and therapy staff.
- Ensure patient safety, establish goals, and participate in care planning meetings; maintain compliance with regulatory and facility policies.

Requirements
- Active physical therapy license in New Hampshire (or eligibility for licensure)
- Doctor of Physical Therapy (DPT) degree or equivalent
- Minimum 1 year of physical therapy experience in a clinical setting (preferred)
- Experience in skilled nursing facility or geriatric rehabilitation preferred
- Strong communication and documentation skills; ability to work both independently and as part of an interprofessional team
