Geriatric Care Manager — Flexible Hybrid, Client‑Focused
Unicity Care Management
Unicity Care Management seeks a Care Manager to handle a small private-pay client caseload, coordinating services and monitoring needs in clients’ homes. The CM will also develop plans of care with families and interact with referral sources to drive new client referrals. Responsibilities include conducting comprehensive assessments, coordinating resources, tracking time for billing, and providing emotional support as needed.
